Enrolling as a new patient

Our books are currently open to those people who are new to the district, and are not currently enrolled at a GP practice in Ashburton.

Fiona Smith is your contact for all new patient enquiries. Please email Fiona with the following information:

• Full name including middle name
• Date of birth
• Address
• Phone numbers
• Name of previous medical practice
• For NZ born residents or citizens, a copy of either passport or birth certificate

• For non-New Zealanders a copy of passport, including visas (work visas, interim visas, proof of New Zealand citizenship or residency).
• Please note, a mandatory enrolment appointment with the Practice Nurse is required for all new patients aged 16 years and older.
• Fees will apply to all consultations.  Payment is expected on the day.
• In some cases, an appointment with our Clinical Pharmacist is also required.
